Pastor W-2

Hello,
Our Pastor has been on the Portico Silver+ Plan and part of that is that employer paid $75 for HSA. I entered the item as print on W-2 box code W and it does not print.
There is also an E code which appears in box 12a for retirement. But box 12b does not populate with the W code.
Also, there is a Supplemental Life amount through Portico of $5/mo - his item should be "After Tax Deduction" and not on the W-2, correct?
